Extended fasting under medical supervision, from a modified low-calorie protocol through to water only. By day three or four most people report a marked change in mental state: flat, clear, and oddly unbothered. It is also the practice on this page with the most serious physiological consequences if done badly.

Two traditions, and they are not the same. The European clinical model uses a very low calorie intake with broth and juice, keeps you walking and treated daily, and has a century of institutional practice behind it. The North American model at TrueNorth is water only, in bed, with daily physician monitoring and blood work.

The altered state is real and under-discussed. Prolonged fasting shifts cognition and mood in a way people frequently describe as a lifting, and religious traditions from Ramadan to Lent to the wilderness fast have organised around exactly that. It arrives without anything ingested and without a facilitator.

Do not improvise this. The dangerous part is not the fast, it is the refeeding, and the people most likely to be harmed are the ones who did it alone after reading about it.

What happens

  1. Days 1 to 2Hunger, headache, irritability. Caffeine withdrawal makes this considerably worse for most people.
  2. Days 3 to 4Hunger largely stops. Ketosis established. Energy low, mind commonly reported as clear and flat.
  3. Days 5 onwardStable but weak. Daily monitoring of blood pressure, electrolytes and heart rhythm at any competent facility.
  4. RefeedingRoughly half the length of the fast, supervised. This is the phase that kills people who do it alone.
