MDMA Therapy for PTSD: History, Trials and the FDA Decision
How MDMA went from 1970s therapy rooms to Schedule I to phase 3 trials, how the protocol worked, why the FDA declined it in 2024, and the lawful options as of October 2026.
What this article covers
This is the history and the regulatory story of MDMA-assisted therapy: where it came from, how the trial protocol worked, why the FDA did not approve it in 2024 and what that leaves patients with now. For the current evidence, trial by trial, and the lawful routes kept up to date, see MDMA for PTSD: the evidence and the FDA decision. That page is the place to check status; this one explains how things got there.
In brief: two phase 3 trials found that MDMA plus therapy reduced PTSD symptoms more than placebo plus the same therapy. (Nature Medicine, 2021; Nature Medicine, 2023) The FDA still declined the application and asked for another phase 3 trial. (Lykos Therapeutics (press release), 9 Aug 2024; eCFR (Office of the Federal Register), current)
Before 1985: a drug therapists used quietly
MDMA is older than its reputation. A chemist at the German company Merck made it as a by-product while working on a different compound, and Merck filed a patent on the method in 1912, but no pharmaceutical testing followed. (Der Nervenarzt, 2006)
Its therapeutic history begins in the 1970s. The chemist Alexander Shulgin re-synthesised MDMA and tried it himself in September 1976; in 1977 he gave it to the psychotherapist Leo Zeff, who used it as an adjunct to psychotherapy and introduced it to other therapists. (Addiction, 2010) In 1978 Shulgin and David Nichols published the first paper on its effects in humans. (Addiction, 2010)
At the same time MDMA had a separate life as a street drug. Underground chemists made it from the late 1960s as a legal alternative to MDA, recreational use spread across the US in the early 1980s, and in 1985 it was scheduled under the Controlled Substances Act. (Journal of Psychoactive Drugs, 2016) That ended open therapeutic use. The Multidisciplinary Association for Psychedelic Studies (MAPS) was founded in 1986 in direct response to the scheduling, with the aim of taking MDMA through FDA trials. (Journal of Psychoactive Drugs, 2014) MDMA is still a Schedule I substance under federal law. (eCFR (Office of the Federal Register), current)
The long road back into trials
The first randomised, placebo-controlled pilot trial of MDMA-assisted psychotherapy for PTSD was small: 20 people whose chronic PTSD had not responded to therapy or medication. Ten of 12 responded with MDMA against 2 of 8 with placebo. (Journal of Psychopharmacology, 2011) Further phase 2 studies followed, organised by MAPS Public Benefit Corporation, a subsidiary wholly owned by MAPS. (Psychopharmacology, 2024) The phase 3 investigators received funding from MAPS PBC for the study, and the application the FDA reviewed in 2024 was filed by Lykos Therapeutics. (Nature Medicine, 2021; U.S. Food and Drug Administration, 26 Jul 2024)
How the protocol worked
The phase 3 protocol was a fixed course of therapy, not a prescription. (ClinicalTrials.gov, 2018)
- Preparation. Three preparatory sessions before any dose. The FDA's summary of the treatment manual says these were for taking a history, building a working relationship and preparing the participant for anxiety or physical tension as traumatic memories surfaced. (US Food and Drug Administration, 4 Jun 2024)
- Dosing sessions. Three experimental sessions, three to five weeks apart. Each began with 80 mg or 120 mg of MDMA (or placebo) and a supplemental half dose 1.5 to 2 hours later, these being trial doses given under supervision. (ClinicalTrials.gov, 2018) Participants were monitored by two therapists for about eight hours, and most stayed overnight at the study site. (US Food and Drug Administration, 4 Jun 2024)
- Integration. Three integrative sessions after each dosing session, nine in all. (US Food and Drug Administration, 4 Jun 2024)
The therapists' stance in dosing sessions was meant to be supportive and non-directive, following the participant's lead. (US Food and Drug Administration, 4 Jun 2024) Participants also came off their psychiatric medications before treatment. (Nature Medicine, 2021) That was a trial requirement under medical supervision; anyone taking an antidepressant should not stop it without their prescriber. In a study of healthy volunteers, the antidepressant citalopram markedly reduced most of MDMA's psychological effects. (Neuropsychopharmacology, Liechti ME, Baumann C, Gamma A, Vollenweider FX, 2000; Psychopharmacology (Berl), Sarparast A, Thomas K, Malcolm B, Stauffer CS, 2022)
The phase 3 trials, as trial facts
MAPP1 enrolled 90 adults with severe PTSD, including people with dissociation, depression, a history of substance use disorder or childhood trauma. Among completers, clinician-rated PTSD scores (CAPS-5) fell by 24.4 points with MDMA and 13.9 with placebo, both groups receiving the same therapy. (Nature Medicine, 2021) The authors reported no adverse events of abuse potential, suicidality or QT prolongation with MDMA. (Nature Medicine, 2021)
MAPP2 enrolled 104 adults with moderate or severe PTSD. Scores fell by 23.7 points with MDMA and 14.8 with placebo plus therapy. (Nature Medicine, 2023) Severe treatment-emergent adverse events occurred in 5 of 53 people on MDMA and 2 of 51 on placebo, with no deaths and no serious adverse events. (Nature Medicine, 2023)
Both trials found real differences on their primary measure. The question the FDA had to answer was how much of that difference the drug caused.
The 2024 advisory committee and the complete response letter
On 4 June 2024 the FDA's Psychopharmacologic Drugs Advisory Committee reviewed the application. It voted 9 to 2 that the data did not show the drug was effective, and 10 to 1 that its benefits, with the FDA's proposed risk programme, did not outweigh its risks. (U.S. Food and Drug Administration, 26 Jul 2024) On 9 August 2024 Lykos announced that the FDA had issued a complete response letter: it could not approve the drug on the data submitted and had asked for an additional phase 3 trial. (Lykos Therapeutics (press release), 9 Aug 2024; eCFR (Office of the Federal Register), current)
The criticisms
The FDA's own briefing document for the committee set out most of the concerns. (US Food and Drug Administration, 4 Jun 2024)
- Functional unblinding. The trials were designed as double-blind, but in a post-study survey about 90% of people given MDMA and 75% given placebo correctly guessed which they had received. (US Food and Drug Administration, 4 Jun 2024)
- Expectation bias. The reviewers wrote that people who believed they had the active drug may have expected to improve, and those on placebo may have done worse from disappointment, and that it was reasonable to assume this affected the results to some extent. (US Food and Drug Administration, 4 Jun 2024)
- The therapy's share of the effect. Every arm included psychotherapy, so the briefing said the contribution of the therapy had not been characterised, and the content of integrative sessions was not standardised in the manuals. (US Food and Drug Administration, 4 Jun 2024)
- Safety data gaps. The briefing called the cardiac profile not well characterised, noted significant rises in blood pressure and pulse, said liver tests were not collected in the phase 3 studies, and said effects such as euphoria were not recorded, a gap that limited the assessment of abuse potential. (US Food and Drug Administration, 4 Jun 2024)
Conduct at one site became part of the story too. In its November 2024 issue, the journal Psychopharmacology retracted three papers drawn from the phase 2 programme, citing protocol violations amounting to unethical conduct at the MP4 study site that the authors had known about but not disclosed, and incomplete declaration of competing interests. (Psychopharmacology, 2024; Psychopharmacology, 2024; Psychopharmacology, 2024) Several authors said they disagreed with the retraction. (Psychopharmacology, 2024)
None of this means the trial results were invented. It means the FDA judged that the design could not separate the drug's effect from expectation and therapy well enough to approve it.
What happens next, and what patients can lawfully do now
As of October 2026, no MDMA product is FDA approved, and the FDA's request for another phase 3 trial stands. (Lykos Therapeutics (press release), 9 Aug 2024; eCFR (Office of the Federal Register), current) Research continues: the US Department of Veterans Affairs announced in December 2024 that it would fund a study of MDMA-assisted therapy for veterans with both PTSD and alcohol use disorder, and said it strongly discourages self-medicating with psychedelics. (U.S. Department of Veterans Affairs, 3 Dec 2024)
The lawful options are narrow.
- Clinical trials. Search recruiting PTSD trials. A ClinicalTrials.gov listing means the sponsor registered the study, not that the government evaluated it. (U.S. National Library of Medicine (NIH), Oct 2023) Ask the team what taking part costs and what expenses are reimbursed. (National Institute on Aging (NIH), current page)
- Australia. Since 1 July 2023, psychiatrists with Authorised Prescriber approval may prescribe MDMA for PTSD; no MDMA product has been evaluated or registered by the regulator, and MDMA stays prohibited for any other use. (Department of Health, Victoria (Australia))
- Approved PTSD treatment now. The FDA's briefing lists psychotherapy and medication as current options, with two antidepressants, paroxetine and sertraline, approved for PTSD. (US Food and Drug Administration, 4 Jun 2024) Talk to your clinician before changing anything you take.
Illicit MDMA is not the trial drug in a trial setting. In healthy study subjects, MDMA alone raised systolic blood pressure above 160 mmHg in a third of cases, and it has caused fatal serotonin toxicity combined with the antidepressant moclobemide. (International Journal of Neuropsychopharmacology, Hysek CM, Simmler LD, Schillinger N, et al., 2014; Journal of Psychopharmacology, Vizeli P, Liechti ME, 2017; Forensic Science International, Pilgrim JL, Gerostamoulos D, Woodford N, Drummer OH, 2012; Journal of Analytical Toxicology, Pilgrim JL, Gerostamoulos D, Drummer OH, 2011) Check any combination with the interaction checker, and see the MDMA substance page for its pharmacology.

Questions and answers
- Why did the FDA reject MDMA therapy for PTSD?
- In August 2024 the FDA issued a complete response letter saying it could not approve Lykos's application on the data submitted and asked for another phase 3 trial. Its briefing for the advisory committee had flagged functional unblinding, since most participants guessed whether they had MDMA, possible expectation bias, the uncharacterised role of the therapy, and gaps in cardiac, liver and abuse-potential data.Sources: Lykos Therapeutics (press release), 9 Aug 2024; eCFR (Office of the Federal Register), current; US Food and Drug Administration, 4 Jun 2024MDMA for PTSD: the evidence and the FDA decision
- How many MDMA sessions were used in the phase 3 trials?
- Three. Each MDMA session, three to five weeks apart, began with 80 mg or 120 mg and a supplemental half dose 1.5 to 2 hours later, given under supervision. The sessions were preceded by three preparatory sessions and each was followed by three integrative therapy sessions, nine in all.
