The Future of Psychedelic Medicine: What to Expect
What is pending as of October 2026, from psilocybin and LSD applications to MDMA after its rejection, and the open problems that will decide how far psychedelic medicine goes.
Where things stand in October 2026
Psychedelic medicine is still mostly a research field. Psilocybin and psilocyn are Schedule I controlled substances, and no psilocybin medicine is FDA approved. (eCFR (Office of the Federal Register), current; U.S. Food and Drug Administration, Jul 2026) MDMA is also Schedule I and no MDMA product is approved. (eCFR (Office of the Federal Register), current; Lykos Therapeutics (press release), 9 Aug 2024) The closest approved medicine is esketamine (Spravato), approved in March 2019 and given only in certified settings. (U.S. Food and Drug Administration, Drugs@FDA, 5 Mar 2019; U.S. Food and Drug Administration, Drugs@FDA, 2026; U.S. Food and Drug Administration, REMS@FDA, 24 Jun 2026; U.S. National Library of Medicine, DailyMed (Janssen label), 24 Sep 2026)
This article looks ahead: what is pending, and which problems will decide how far these treatments go. For the current evidence on each condition, the condition pages are the place to start: psilocybin for depression, MDMA for PTSD and ketamine and esketamine. Nothing here predicts when, or whether, any product will be approved.
What is pending with regulators
Psilocybin for treatment-resistant depression. COMPASS Pathways said on 5 August 2026 that the FDA had granted a rolling submission and review of its application for COMP360, a synthetic psilocybin, that some modules were already submitted, and that it expected to complete the submission in the fourth quarter of 2026. That is a company statement about a pending application, not an approval. (COMPASS Pathways plc, via SEC EDGAR, 5 Aug 2026) The FDA finalised its guidance on designing clinical trials of psychedelic drugs in July 2026. (eCFR (Office of the Federal Register), current; U.S. Food and Drug Administration, Jul 2026)
LSD for generalised anxiety. Mind Medicine (MindMed) changed its name to Definium Therapeutics in January 2026. (Definium Therapeutics (SEC filing), 9 Jan 2026) Its product, DT120, is an orally disintegrating tablet of LSD (lysergide). (Definium Therapeutics, 8 Sep 2026) In the Voyage trial, 214 adults were randomised to a single 100 microgram dose or placebo; at week 12, anxiety scores (HAM-A) fell 11.6 points with DT120 and 6.2 with placebo. (Definium Therapeutics, 12 Aug 2026; ClinicalTrials.gov, 2026) In the second trial, Panorama, 245 people were enrolled; the 100 microgram dose beat placebo by 5.1 points at week 12, and the company says it plans to file a new drug application in the first half of 2027. (Definium Therapeutics, 14 Sep 2026; ClinicalTrials.gov, 2026) The company also reported an 8.1-point placebo-adjusted improvement in depression scores (MADRS) at week 6 in a phase 3 depression trial, and the FDA has given DT120 Breakthrough Therapy designation for both anxiety and major depression. (Definium Therapeutics, 8 Sep 2026) Breakthrough designation is meant to speed development; it is not a marketing approval. (COMPASS Pathways via PR Newswire, 2018; Usona Institute (press release), 2019) These figures come from company press releases; full peer-reviewed papers had not appeared when this was written.
MDMA for PTSD. In June 2024 an FDA advisory committee voted 9 to 2 that Lykos's data did not show effectiveness. (U.S. Food and Drug Administration, 26 Jul 2024) In August 2024 the FDA issued a complete response letter and asked for an additional phase 3 trial. (Lykos Therapeutics (press release), 9 Aug 2024; eCFR (Office of the Federal Register), current) The US Department of Veterans Affairs has since funded a study of MDMA-assisted therapy for veterans with PTSD and alcohol use disorder, about $1.5 million over five years. (U.S. Department of Veterans Affairs, 3 Dec 2024) The MDMA route now depends on new trial data rather than on a decision already in the queue.
Programmes that already exist outside the FDA route
Some access exists now, under state or national law rather than FDA approval. Oregon licenses psilocybin service centres for adults 21 and over, with use only on site under a facilitator. (Oregon State Legislature, current edition; Oregon Health Authority / Oregon Secretary of State, 2025) In 2025, 5,935 clients received services at licensed centres. (Oregon Health Authority, 2026) Colorado issued its first healing centre licence on 31 March 2025. (Colorado General Assembly, 23 May 2023; Colorado Department of Revenue, Natural Medicine Division, 2 Apr 2025) New Mexico's Medical Psilocybin Act is a medical programme, not an adult-use one, with a statutory deadline of 31 December 2027. (New Mexico Legislature, 2025; New Mexico Department of Health, current page) State licensing does not make psilocybin federally lawful. (Legal Information Institute (Cornell) reproducing the U.S. Code, current text) The legal psilocybin guide lists the licensed centres.
Abroad, Australia has let authorised psychiatrists prescribe psilocybine for treatment-resistant depression since July 2023, and the Czech Republic has allowed psychiatrist-supervised medical psilocybin in Ministry of Health hospitals since 1 January 2026. (Therapeutic Goods Administration, 2023; Therapeutic Goods Administration, current page; Federal Register of Legislation (Australian Government), 30 Sep 2026; e-Sbirka, Ministry of the Interior of the Czech Republic, 5 Aug 2025) In Germany, the medicines regulator approved a compassionate-use programme in July 2025 that lets individual adults with treatment-resistant depression receive psilocybin at two clinics, in justified exceptional cases. (Central Institute of Mental Health (ZI) Mannheim, 31 Jul 2025; The Lancet Psychiatry 13(2):91-93 (online 18 Sep 2025), Feb 2026) These are narrow medical routes, not retreat permissions.
The evidence problems still open
Blinding and expectancy
An inactive placebo rarely feels like a strong psychedelic, so participants can often guess which group they are in. A 2021 review found that psychedelic trials had generally not reported participants' expectations or whether blinding worked, and argued that effect sizes are likely overestimated as a result. (Expert Review of Clinical Pharmacology, 2021) The Institute for Clinical and Economic Review (ICER), which assesses the evidence and value of medicines, rated the public MDMA evidence insufficient in June 2024, citing functional unblinding among its concerns. (Institute for Clinical and Economic Review, 27 Jun 2024) Some newer designs try to address this: Panorama included a 50 microgram arm "to help mitigate functional unblinding". (Definium Therapeutics, 14 Sep 2026)
Smaller effects in larger trials
Early trials were small and often used waitlist controls. In a 2021 trial of 27 people, depression scores in the psilocybin group were about 8 against 23.5 in the waiting group (Cohen d about 2.5). (JAMA Psychiatry, 2021) In the 233-person COMPASS phase 2 trial, the 25 mg dose beat a 1 mg dose by 6.6 points at week 3, and the 10 mg dose did not differ from 1 mg. (New England Journal of Medicine, 2022) The scales and comparators differ, so the numbers are not directly comparable, but the pattern matters. A larger trial can still show a clear difference: in 104 adults, psilocybin beat an active placebo (niacin) by 12.3 points at day 43. (JAMA, 2023) MDMA shows the shrinkage on one scale: the effect size was 0.91 in the first phase 3 trial and 0.7 in the second. (Nature Medicine, 2021; Nature Medicine, 2023)
How long benefits last
Durability is not settled. In the COMPASS trial, sustained response at 12 weeks was not clearly supported for the 25 mg group. (New England Journal of Medicine, 2022) Long follow-ups are encouraging but uncontrolled: of 18 people who completed five-year follow-up after a small psilocybin trial, 67% were in remission. (Journal of Psychedelic Studies, 2025) In the 104-person trial, more people had a sustained response with psilocybin, but not more sustained remission. (JAMA, 2023) Whether people will need repeat doses, and how often, is a question the approved-drug era will have to answer.
Safety monitoring at scale
Controlled trials screen people carefully and watch them closely. A 2024 meta-analysis found nine serious adverse events across more than 1,000 administrations in such settings, and noted that retreats do not necessarily replicate that screening and monitoring. (Psychiatry Research, 2024) Adverse events are still common: 77% of participants in the COMPASS trial had at least one, and suicidal ideation, behaviour or self-injury occurred in all dose groups. (New England Journal of Medicine, 2022) In Panorama, 94.8% on the 100 microgram dose reported a treatment-emergent adverse event, against 62.9% on placebo, with no drug-related serious adverse events. (Definium Therapeutics, 14 Sep 2026)
Esketamine shows what a monitoring framework looks like once a drug is approved: a restricted programme, certified settings, and at least two hours of observation after every dose. (U.S. Food and Drug Administration, REMS@FDA, 24 Jun 2026; U.S. National Library of Medicine, DailyMed (Janssen label), 24 Sep 2026) What monitoring regulators would require for psilocybin or LSD is not yet public. Oregon gives one picture of real-world use outside trials: in 2025, facilitators or centres contacted emergency services 13 times, and 258 people were denied services, 147 of them as ineligible. (Oregon Health Authority, 2026) The screening checklist covers the questions to ask before any session.
Cost, therapist time and insurance
The honest answer on cost is that the data are thin. Oregon centres are not required to report what sessions cost. (Oregon Health Authority, 2026) We found no official source for what off-label ketamine infusions cost, which is why the ketamine therapy guide gives no figure. For esketamine, ICER judged the announced list price of $32,400 a year low value for money. (Institute for Clinical and Economic Review, 20 Jun 2019) For MDMA, ICER calculated no price benchmark because no price had been announced. (Institute for Clinical and Economic Review, 27 Jun 2024)
Therapist time is the other cost driver. Psychedelic sessions last hours, and group formats are one way services spread that time: Oregon reported 4,628 individual and 747 group administration sessions in 2025. (Oregon Health Authority, 2026) We found no official source describing insurance coverage for psilocybin, LSD or MDMA treatment, so ask your insurer and the provider in writing before starting anything.
Non-hallucinogenic analogues
One line of research asks whether the trip is needed at all. In 2021, researchers reported tabernanthalog, a non-hallucinogenic analogue of ibogaine that promoted neural plasticity and reduced alcohol- and heroin-seeking in rodents. (Nature, 2021) That is animal work, not evidence of benefit in people.
The underlying question is contested. One 2021 paper argued that the subjective effects of psychedelics may not be necessary for lasting benefit. (ACS Pharmacology & Translational Science, 2021) A companion paper from Johns Hopkins argued that they are necessary and account for most of the benefit. (ACS Pharmacology & Translational Science, 2021) Human trials will have to settle it.
Who the research includes
Trial results only generalise to people like those enrolled. A 2018 review of 18 psychedelic studies found that 82.3% of 282 participants were non-Hispanic White and 2.5% African American. (BMC Psychiatry, 2018) Access can also narrow at the local level: in Oregon, 25 counties and 26 cities banned psilocybin businesses in 2022. (Oregon State Legislature, current edition; Oregon Health Authority, current page; OPB, 7 Nov 2024) Cost, geography and eligibility rules will shape who benefits as much as any approval decision.
How to read the next headline
- A company press release is not a peer-reviewed paper, and a filing is not an approval.
- Check the comparator: waitlist, inactive placebo, low dose or active drug.
- Look for durability beyond a few weeks and for how adverse events were counted.
- Use ClinicalTrials.gov listings to see what is recruiting; a listing is not an endorsement. (U.S. National Library of Medicine (NIH), Oct 2023)

Questions and answers
- Which psychedelic medicines are waiting for FDA decisions?
- As of October 2026, COMPASS Pathways says it expects to complete its rolling application for COMP360 psilocybin in treatment-resistant depression in the fourth quarter of 2026, and Definium Therapeutics says it plans to file for its LSD tablet DT120 in generalised anxiety in the first half of 2027. Neither is approved, and MDMA needs a further phase 3 trial after the FDA's 2024 complete response letter.Sources: COMPASS Pathways plc, via SEC EDGAR, 5 Aug 2026; Definium Therapeutics, 14 Sep 2026; Lykos Therapeutics (press release), 9 Aug 2024; eCFR (Office of the Federal Register), currentPsilocybin for depression: the evidence
- Is LSD being tested as a treatment for anxiety?
- Yes. Definium Therapeutics, formerly MindMed, ran two phase 3 trials of a single 100 microgram dose of DT120, an LSD tablet, in generalised anxiety. In Voyage (214 adults) anxiety scores fell 11.6 points against 6.2 on placebo at week 12; in Panorama (245 enrolled) the difference was 5.1 points. - Will health insurance pay for psychedelic therapy?
- There is little to go on yet. No psilocybin, LSD or MDMA medicine is approved, Oregon's licensed centres do not have to report prices, and ICER set no price benchmark for MDMA because none had been announced. For approved esketamine, ICER judged the $32,400 list price low value. - What is a non-hallucinogenic psychedelic?
- It is a compound designed to keep some of a psychedelic's biological effects without the trip. Tabernanthalog, an analogue of ibogaine reported in 2021, promoted neural plasticity and reduced alcohol- and heroin-seeking in rodents. Researchers disagree on whether the subjective experience is needed for lasting benefit, and human evidence is still lacking.Sources: Nature, 2021; ACS Pharmacology & Translational Science, 2021; ACS Pharmacology & Translational Science, 2021
- Why is blinding a problem in psychedelic trials?
- Participants can usually tell whether they received an active psychedelic, so expectations can inflate the measured difference from placebo. A 2021 review found psychedelic trials rarely reported expectancy or blinding success, and ICER cited functional unblinding when it rated the MDMA evidence insufficient in 2024.Sources: Expert Review of Clinical Pharmacology, 2021; Institute for Clinical and Economic Review, 27 Jun 2024
